Certain regulations MUST BE satisfied and records MUST BE presented in order for a student to be enrolled in JISD.  These include:

PROOF OF SOCIAL SECURITY NUMBER (Social Security Card)

PROOF OF IDENTIFICATION OF STUDENT (Birth Certificate, Passport, Driver’s License, Adoption, Etc.)

PROOF OF RESIDENCE (Tax Statement for current year OR Voter Registration Card for current year OR Rental lease signed/dated by both Leasee and Leasor and Including Tax Statement of  Property Owner and Listing Names of parent/guardian & child(ren) living in residence)

IMMUNIZATION RECORDS (Dpt, Measles, Mumps, Rubella, Polio, Haemophilus, Influenza Type B, Tuberculin Test)

WITHDRAWAL / OTHER RECORDS (That You Have From Your Previous School (Including SS# Or Alternate ID). JISD Will Send For The Official School Records.)

DIVORCE DECREE- IF APPLICABLE (With Custodial Parent/ Guardian  Listed And Dated)

AUTHORIZATION AGREEMENT FOR NON-PARENT/RELATIVE, VOLUNTARY CAREGIVER (Power Of Attorney)

**IF A STUDENT IS TO LIVE WITH SOMEONE OTHER THAN A NATURAL PARENT OR LEGAL GUARDIAN, THAT PERSON MUST BE RESPONSIBLE FOR SIGNING ALL ENROLLMENT DOCUMENTS IN THE SCHOOL OFFICE.

Students MUST LIVE in JISD To Attend School at JISD.  Students are required to bring proof of residence in the district.  

NOTE:  IT IS A VIOLATION OF STATE LAW TO KNOWINGLY FALSIFY SCHOOL ENROLLMENT DOCUMENTS.
